Registration Guidelines:

1) Please complete all purchases (registrations, Expo, etc.) as a single transaction.  

• Register individually everyone in your household, or anyone whom you are paying for, BEFORE completing your purchase.

2) Children two years and under attend all events free.    Please list their names in the space provided, but do not register them.

3) Register children three through eleven at the child rate.  Please select the age of each child registered. 

4) Register children 12 through 17 at the teen rate.  Please select the age of each teen registered.

5) Complete all required information for each registrant.  

ALERT:  Do not complete the following two sections:  T-shirt Color and Registration Bag. These two fields are for use by the form administrator only.  These sections will auto-complete depending on your selections during registration.

• VEGETARIAN OPTIONS:
  1. Thursday LAB Explosion! (Love, Laughter & Amusement Blastoff) - 6 PM (cole slaw, beans, and/or garden salad)
  2. Friday FLAME Gala (Family Legacy Awards and Member Extravaganza) - 7 PM - Quinoa Stuffed Pepper
  3. Saturday Morning BAR (Breakfast Buffet, Auction and Recreation). Please choose from vegetarian-friendly items on the buffet. 
• PESCATARIAN OPTIONS:
  1. NONE available during Thursday Hospitality LAB. Please choose from other palatable items.
  2. Friday FLAME Gala - Salmon
  3. Saturday Morning BAR Breakfast Buffet: NONE available. Please choose from buffet items that do not compromise your pescatarian preferences.

Every attempt is made to accommodate vegan/vegetarian and pescatarian preferences. Vegan /vegetarian / pescatarian availability during dinner is based on selections made on this registration form. LEMFHA has already submitted dietary requests to the caterer and preferences cannot be added or deleted during the event.
